A tablet has to go through the gut wall and then straight through the liver before any of it reaches the rest of you. The liver takes a cut on the way past, and what is left is the bioavailable fraction.

It does. Straight into the blood, nothing taken off, which is why the oral dose of the same drug is the larger of the two.
